Our primary services are family-centered geriatric assessment and care management. A Wellspring geriatric assessment is a comprehensive view of the medical, psychological, functional, social, and family problems faced by older adults. Geriatric assessments help avoid costly medical care while enhancing functional ability and independence. This assessment is the basis for evaluating and improving the overall quality of life for older adults and their families. Initially, we will conduct a home visit with our physician-social worker team. This first visit includes a physical and functional exam, medication review, sensory screening, a psy- chological evaluation, and an in-depth discussion about concerns and expectations. Next a comprehensive written profile and care plan-presented in non-technical, easily understood language- is prepared. This report specifically addresses diagnoses, abilities, risk levels and resources. Problems and solutions are prioritized with an eye toward the future. We will review and discuss this assessment with all involved, so that they may act with confidence on any or all of its recommendations.
